Huron Perth & Area Ontario Health Team makes history with first-ever collaborative accreditation, achieving Exemplary Standing from Accreditation Canada

 

May 22, 2024 – Huron Perth & Area, Ontario – The Huron Perth & Area Ontario Health Team (HPA-OHT) is proud to announce that our collaborative of 10 organizations has received the esteemed Accredited with Exemplary Standing from Accreditation Canada. This marks the first time in Canada that a collaborative has been evaluated and accredited as an integrated system rather than as individual organizations. This prestigious recognition underscores the exceptional quality of care and patient safety provided by the HPA-OHT, setting a new standard for healthcare in our region.

A Unique Collaborative Achievement:

What sets this accreditation apart is that it is awarded to a collective of ten organizations, including nine of our member organizations and the HPA-OHT itself. This collaborative effort demonstrates the power and effectiveness of unified healthcare delivery, highlighting how different organizations working together can achieve extraordinary results. The HPA-OHT Accreditation Collaborative includes the Alzheimer Society Huron Perth, Clinton Family Health Team, Community Living North Perth, Huron Health System, Huron Perth Healthcare Alliance, Knollcrest Lodge, Listowel-Wingham & Area Family Health Team, ONE CARE Home and Community Support Services, West Perth Village, and the Huron Perth & Area Ontario Health Team.

This designation signifies that the HPA-OHT Accreditation Collaborative meets and exceeds the rigorous performance standards set by Accreditation Canada.
