Health Records

Service Summary

The Health Records Department provides transcription services, statistics for utilization management review, chart completion, medical/legal correspondence, coding and abstracting, ongoing quality assurance activities, research audits, archiving of records, and control of patient records. The hospital shares a database (patient record) with the Huron Perth Healthcare Alliance.


The Health Records Department is responsible for the release of patient health information, including providing copies of health records to authorized persons.


Health Records acts as Ontario Telemedicine Network (OTN) site coordinator to facilitate remote meetings.


Transcription services are available to staff physicians and departments. The hospital shares dictation systems in the region and has implemented voice recognition.


Coding and abstracting of emergency, surgical and inpatient medical records provides the Ministry of Health with accurate comparable data on diseases and interventions, standardized by the World Health Organization. This enables hospital funding as well as providing statistical health information for the demographic area.


Utilization Management measures, seeks to understand, and when appropriate, reduces inappropriate hospital use while maintaining quality of care. A key role on the FLO Collaborative team aids in monitoring ALC (Alternate Level of Care) patients, education, documentation and statistics. Clinical data/statistical information is collected and analyzed with report generation to the Utilization team as well as the Board Quality Assurance Committee.


Health Records is working towards a fully integrated electronic Health record.
